Dr. McCarthy manages clinical records and billing with an Electronic Medical Record (EMR) system hosted by a company named Luminello.
All patients must have an active portal account, but Dr. McCarthy will tell you when you should complete this form.
Once you submit this form you will receive an invitation via email from luminello.com to set up your portal. You will also receive an email from operations@catherinemccarthymd.com with portal setup instructions.
Your portal will not be active until demographic and payment details are entered and the three administrative documents have been electronically signed.
Your patience after submitting a portal request is appreciated. The submitted information is not processed automatically but requires staff to validate and create the portal invitation.
